なんだかとってもわかりにくいマニュアルだが、SYNOPSYSに

$mailer = new Mail::Mailer $type, @args;

とあってあとの方のARGUMENTSってとこに

"new" can optionally be given a $command and $type. $type is one
"sendmail", "mail", ... given above. The meaning of $command depends
on $type.

と書いてあるのでなんとなく

my $mailer = Mail::Mailer->new('sendmail', "$sendmail");

とやれって意味じゃないかと思う。試してないのではずしてたらスマソ。
